Education, Health and Care Plans (EHCPs)
At Holbrook School for Autism, every pupil has an EHCP at the heart of their personalised learning. These plans outline each pupil’s strengths, needs, and outcomes across education, health, and social care.
We use each EHCP as a living document—guiding teaching, support, and development as our pupils’ needs evolve.
Annual Reviews
An Annual Review meeting is a statutory process that looks at the needs, provision, and outcomes in a pupil’s Education, Health and Care Plan (EHCP) to decide whether any changes are needed.
When and how do these take place?
They are held every year, and families and key professionals contribute—either in school or virtually, depending on what works best. During the meeting, we review the needs, provision, and outcomes set out in your child’s Education, Health and Care Plan (EHCP) and agree any changes needed.
What do I need to do?
You will be contacted by the school with a proposed date and time for your child’s Annual Review meeting. You’ll be asked to share your views and provide details of any professionals involved with your child who should be invited. This information is submitted through EHC Hub (for Derbyshire and Nottinghamshire County Councils) or via electronic forms.
Before the meeting, you’ll receive the school’s Annual Review report along with any reports provided by professionals.
What happens at the meeting?
Your child’s teacher will lead the meeting, welcoming everyone and explaining their role in supporting your child. Together, you’ll review the Annual Review report, discuss progress towards EHCP outcomes, and share any updates from school staff, families, and professionals.
All attendees will have the opportunity to contribute their views, ensuring that every voice is heard. By the end of the meeting, the group will agree on any recommended changes or updates to your child’s Education, Health and Care Plan (EHCP) to make sure it continues to meet their needs effectively.
What is the EHC Hub?
The Education, Health and Care Hub (EHC Hub) supports engagement, contributions and collaboration on EHC needs assessments, plans and reviews. What happens after the meeting?
During or following the meeting, your child’s teacher will complete a detailed Annual Review report summarising the discussion, agreed outcomes, and any proposed changes. This report, along with your views and all professional reports, is submitted to the local authority.
The local authority will review the information and decide whether to amend, maintain, or cease your child’s Education, Health and Care Plan (EHCP). You’ll receive notification of this decision within four weeks of the review meeting.
If amendments are agreed, the local authority will issue a draft EHCP, giving you the opportunity to review and comment before the plan is finalised. This information can be accessed on the EHC Hub.
